You can have all three at once. On an overnight flight from London to Tokyo you cross nine time zones (jet lag), sit for around 12 hours in a dry, pressurised cabin (travel fatigue), and lose a night of sleep doing it (sleep debt). The skill is telling them apart, because each clears on its own schedule.

The master clock sits in the suprachiasmatic nucleus (SCN), a small region of the hypothalamus, and it sets the timing of melatonin and cortisol. Cross several time zones quickly and the SCN is still running on home time while the world around you has moved on.

Light is the dominant zeitgeber, the environmental cue that resets the clock, though meal timing and activity contribute too. Travelling east forces the clock to advance, which is harder than the delay required travelling west, so eastward trips usually take longer to clear. What is travel fatigue, and why does cabin altitude drive it?

Travel fatigue is the body’s non-specific response to the accumulated stress of a journey, and unlike jet lag it has nothing to do with time zones. A long drive or train journey can cause it. On a flight, the single largest driver is the cabin environment itself (Janse van Rensburg et al., Sports Medicine 2021).

Airliners pressurise the cabin to the equivalent of 6,000 to 8,000 feet of altitude, not sea level. At that pressure the air holds less oxygen and blood oxygen saturation falls. In a study of 502 passengers published in the New England Journal of Medicine (Muhm et al., 2007), oxygen saturation dropped by up to 4.4 percentage points at a simulated 8,000 feet, and reports of discomfort, tiredness and headache rose as cabin altitude increased.

Cabin humidity sits at around 10 to 20 per cent, drier than most deserts, which speeds dehydration. Add hours of immobility and interrupted sleep, and you land depleted before time zones enter the equation. This is why a daytime flight that crosses only one or two time zones can still leave you flattened: that is travel fatigue, not jet lag, and it lifts within a day or two.

There is a deeper reason to separate them: you can feel recovered while your body is not.
